What Is Form 1095-A?

Where the Marketplace statement fits into tax-time preparation.

Form 1095-A is the Health Insurance Marketplace Statement. It reports information about Marketplace health insurance coverage, including premiums and related figures for the coverage months. It may be needed to prepare a federal tax return and reconcile advance premium tax credits.

Where to find it

People who had Marketplace coverage can generally access the statement through the Marketplace account or contact the Marketplace if they need help locating it. Review the form carefully for covered individuals and months. If information appears wrong, ask the Marketplace about a correction.

How it relates to Form 8962

Form 8962 is used to calculate the Premium Tax Credit and reconcile advance credit payments when required. Not everyone has identical filing circumstances, so follow the IRS instructions for the correct tax year.
