Doctors, Hospitals & Provider Networks

Practical guidance for verifying doctors, hospitals, pharmacies, and network rules.

A provider network is the set of doctors, hospitals, pharmacies, laboratories, and other providers that have an agreement with a plan. Networks can differ among plans offered by the same insurer, so the insurance company name alone is not enough to confirm a provider is in network.

How to verify a provider

  1. Find the full plan name and network listed in the plan documents.
  2. Use the insurer's provider directory for the exact plan.
  3. Call the insurer and the provider's office to confirm current participation.
  4. Ask whether the specific doctor, facility, and service are in network.
  5. Repeat the check before planned care because networks can change.

Out-of-network care

Out-of-network services may cost more or may not be covered, depending on the plan and service. Emergency care has separate protections, but those protections do not mean every out-of-network service is treated as in-network. Check the plan's rules and applicable consumer protections.

Compare the provider network alongside the premium, deductible, prescription formulary, and annual out-of-pocket maximum.
